Knowledge about anaesthesia comes from four quite different activities, each with its own standards of proof. Knowing which activity a given claim came from is most of what it takes to read the literature intelligently.

Four sources of knowledge

Claims in this subject come from the laboratory bench, from measurement in living subjects, from formal comparison between groups, and from the historical record. They are not interchangeable, and confusion between them accounts for a large part of the disagreement in the literature.

The bench

Work at this level asks what a molecule does to a preparation: to a lipid bilayer, to an isolated protein, to a channel in a membrane patch, to a slice of tissue kept alive. Its strength is control, since the concentration is known exactly and everything else can be held constant, which is why every solid statement about targets comes from here. Its weakness is that a preparation is not an organism. The 1984 experiment that redirected the mechanistic literature used a protein from an organism with no nervous system at all, which is precisely what made it decisive about chemistry and silent about brains.

Measurement in the whole subject

The second activity measures something in an intact organism: a trace, a concentration, a reflex, a response to instruction. Its strength is that the system studied is the one that actually matters. Its weakness is that almost nothing of interest can be measured directly. Depth is not measurable, so a correlate is measured instead and called depth. Pain is not measurable, so report or behaviour is measured instead. A great deal of care in this literature goes into the question of how far the correlate can be trusted to stand in for the thing.

Comparison between groups

The third activity compares outcomes between groups treated differently. This is the only method that can establish that one approach produces better results than another, and it carries a specific structural difficulty in this field: the events of greatest interest are rare. Demonstrating a difference in something that happens once in many thousands of cases requires numbers that a single centre cannot produce, which is why so much of the important work is collaborative, why registries matter, and why the literature contains many small studies that were never capable of answering the question they asked. Reading it well means attending to whether a study was large enough to detect what it was looking for.

The historical record

The fourth activity is scholarship on the record itself: establishing what happened, when, and who did what. This is not decoration. The priority disputes of the nineteenth century were argued for decades on the basis of claims that were never checked, and the conventional narrative of the subject was substantially shaped by whoever published first and most insistently. Correcting that record is real work with real consequences for what the field believes about its own development.

How the subject is taught

As a body of knowledge, the subject is normally approached in a fixed order, and the order is not arbitrary. The physical chemistry comes first, because vapour pressure, solubility and partition determine what an inhaled substance can do. Physiology follows, because the effects are effects on breathing and circulation. Pharmacology comes third, since it is the study of how concentration changes over time in a body. The historical literature usually comes last, and often reads better than any of them, because it explains why the subject is organised the way it is rather than merely stating that it is.

The recurring difficulty for anyone reading in the area is that the sources speak with different degrees of confidence about very similar-sounding things. A bench result about a binding site, a correlate reported as depth, and a comparison between groups are three different kinds of claim. Keeping them apart is the whole of critical reading here.
